PAYMENT DETAILS
The participation fee for the master class on Human Factors and Safety is € 2.095,- excl. VAT. The fee includes all lunches, welcome drinks on Monday and a team dinner on Thursday, as well as your own copy of Sidney’s books "Drift into Failure" and the 2014 edition of "The Field Guide to Understanding Human Error”. The fee does not include hotel costs and transportation. After receiving this registration form the Amsterdam University of Applied Sciences will send you an invoice. Your registration for this event will be completed when your payment is received by Amsterdam University of Applied Sciences."
GENERAL CANCELLATION POLICY FOR EVENTS
Substitution: If, after booking an event you are unable to attend, you are welcome to nominate someone else to attend in your place at no additional cost. Please advise substitutions via email so a correct name tag will be available at the registration desk. Refunds: Cancellation more than 30 days before the event: 90% refund (Please note that we deduct 10% for covering administration costs.) Cancellation less than 30 days, but more than 14 days: 75% refund Cancellation less than 14 days, no refund Cancellation must be in writing via email or letter."
